My wife did have some pain on her leg after going through surgery. We could not book for wheel chair in time and had to request only at the counter, on our journey Delhi (T3) 18:20 hrs to Ranchi. We could get great help from the gentleman Mr Khorram and the Lady Supervisor ( I am sorry I am unable to recollect her name) . It was by their sincere help that my wife could get wheel chair assistance.
